Mid-Fairfield Hospice is a partner with Nursing & Home Care and is a non-profit, licensed home care and hospice agency providing quality end-of-life care to patients and their families in our community.
We offer skilled and compassionate care for persons with life-limiting and/or terminal illness so that they may live out their remaining days in comfort with peace and dignity. We also help families cope with emotional, spiritual and medical issues while caring for a loved one.
We recognize that when one person is ill, the whole family is affected. Mid-Fairfield Hospice is concerned with the needs of the entire family and assists individuals and their loved ones through the various stages of end-of-life care.
Services provided include palliative skilled nursing, home health aides, spiritual care, grief and bereavement counseling and volunteer services. Services can be provided to individuals in their home, in a skilled nursing or assisted living facility or in a hospital setting. |
| At Mid-Fairfield Hospice, our mission is to provide a coordinated, comprehensive care program to patients and their families including physical, psycho-social, spiritual and bereavement support to patients and their families in need of end-of-life care. |
